When you receive treatment as a Hilltop client or resident, a record of your visit is made. Typically, this record contains:

- Communication among health professionals involved in your care.
- Legal document describing the care you received.
- Verification for third-party payers.
- Education for health professionals.
- Source of data for medical research and public health oversight.
- Tool for organizational planning, marketing, and quality assessment.

- Healthcare professionals authorized to enter information into your health record.
- All organization departments and units.
- Home care agencies or professionals contracting with Hilltop.
- Volunteers involved in your care.
- All employees, staff, and other personnel.
